The Credit Card is in my name only, separated from spouse, spouse opened mail from credit card company, and filled out enclosed check payable to spouse, then signed name on signature line. Is this legal, and what recourse do I have?

You must call the police and tell them your spouse is passing forged checks. Also, call your credit card company and tell them to stop sending blank checks at that address.
