i need some info. about the clause "Signature of Person Preparing Form if Other Than Above" Do I need to complete this section if I'm filling the form out for my spouse?

It would be completed in the case of child or disables, but in the case of adult, I don't think anyone other really have to complete that section. always try to fill out the form in your own handwriting, to avoid an obvious misrepresentation on the form.
