Default lien forms needed.

We won a judgement in Circuit court against a moving company that damaged our products in the move. They never plans on paying. Circuit Court said that we could file a lien on his bank account but they didn't have any forms on file to do that. plz recommend some place from where I might find this form?
Thanks.
