Default getting late....?

Hello,
In the Dec. 2003, I was injured in a slip and fall on ice accident. I have 3 years to file a claim to request reimbursement for medical care received as a result of the fall. Must the claim be received by the insurer on or before the anniversary of the injury? What is the citation for the relevant statute covering this?

If the Statute of Limitations is 3 years from act or discovery, whichever comes first. It seems that you are likely too late.
