Default "motion to dismiss"?

Hello people,
I recently was served an alias notice of claim on a debt that is almost a decade old. I hope that the SOL on a credit card debt is 6 years old. So, i am looking to file a motion to dismiss based off of this. I got "motion to dismiss" form on the state's website, but dont know how or where to put on my reason for my claim to dismiss? Please help. Thanks

You can put in like 'Statute of Limitations has expired on this alleged debt'.
